Abstrakt: The article discusses how to implement a new care model in the practice of family medicine. It presents lessons learned at the three stages of change: adoption, implementation, and sustaining change. A particular focus is given to the implementation of a delegate model, a team-based approach to care, at Penobscot Community Health Care (PCHC) in Maine. The delegate model aims at reducing provider burnout and improve the quality of care.
