The endodontic treatment of a mandibular molar with aberrant canal configuration suchasC-shapedcanaleonfiguralioncanbe diagnostically and technically challenging. This case report describes the diagnosis and the treatment of a mandibular second molar with a C-shaped canal anatomy which was confirmed by taking a 3-Dimensional Cone Beam Computed Tomography scan. CBCT systems are available that provide small field of view images at low dose with sufficient spatial resolution for applications in endodontic diagnosis, ircalmcnl guidance and post-treatment evaluation. Care that should be taken right from the diagnosis, access opening, bio-mechanical prepa rat ion and three -dimen si onalobtura lion of a mandibular second molar with C-shaped configuration forsuccessful endodont ic therapy is described in thus case report.
