The contribution of ultrasonography in neonatal emergencies.

Abstrakt: Background. Different types of serious illnesses can affect neonates admitted in neonatal intensive care units. Management of lifethreatening pathologic conditions involves ultrasound monitoring and invasive maneuvers performed under ultrasound guidance: thoracocentesis, pericardiocentesis, paracentesis, atrial septostomy, transfontanelar puncture. Matherial and methods. The authors present the cases of two neonates admitted in Târgu-Mureș Regional Neonatal Intensive Care Unit diagnosed with life-threatening pathology. Results. First case: a late preterm newborn with severe bilateral pleural effusion diagnosed by prenatal ultrasound and treated as the initial intervention with pleuroamniotic shunting, was treated postanatally with bilateral chest tube insertion, mechanical ventilation and adequate caloric intake with formula containing medium chain triglycerides. Follow-up consisted of daily chest ultrasound examination. Second case: a late preterm newborn with obstructive cardiogenic shock due to massive pericardic fluid secondary to the malposition of an umbilical vein catheter diagnosed by echocardiographic examination, was treated by performing emergency pericardiocentesis under ultrasound guidance, with favorable outcome.
