Brief Background Developmental anomalies usually influence the morphology of the tooth that causes changes in tooth alignment and appearance. Careful clinical and radiographic examination will help the clinician in diagnosing and treating such cases. Materials and Methods A case of 9 year old female with enlarged maxillary right central incisor, rotated left maxillary central incisor and a mesiodens is presented. Extraction of mesiodens followed by two by four orthodontic treatment for the derotation of left maxillary central incisor and closure of large midline diastema was done. Finally, odontoplasty of the geminated right maxillary central incisor along with esthetic restoration by composite resin was done. Discussion Presence of a geminated tooth with talon's cusp and a mesiodens is a very rare finding. In this case both orthodontic and aesthetic treatment was done to achieve the desired results. Two by four orthodontic treatment provides complete control of anterior tooth position, is extremely well tolerated, requires no adjustment by the patient and allows accurate and rapid positioning of the teeth. So this treatment approach was followed in the described case to attain the aesthetic results. Summary and Conclusions Geminated-taloned teeth and mesiodens can cause a variety of clinical problems that call for early diagnosis. So, complete knowledge of the aetiology and various treatment modalities available contribute to the success of aesthetic rehabilitation of the patient. [ABSTRACT FROM AUTHOR] |
