PURPOSE: To demonstrate the pitfalls of uterine artery embolization for fibroids. BACKGROUND: Uterine artery embolization has been used in the last decade as alternative treatment for uterine fibroids. Rarely sarcomas have been reported of being postembolized. It is not known if the embolization was the trigger event for sarcomatous changes of fibroids or an uterine sarcoma misdiagnosed as benign tumor. CASE REPORT: A 50 year old obese woman underwent uterine artery embolization due to severe uterine bleeding and massive pulmonary embolism. Several months later, she had huge increase of the fibroid size that raised the suspicion of sarcoma. She had total abdominal hysterectomy, bilateral salpigo-oophorectomy, partial excision of small bowel, omentectomy and adjuvant chemotherapy. The patient is alive, one year after the operation. Review of the literature, regarding the patient selection for uterine fibroids for embolization. RESULT‟s: There is not effective algorithm to predict malignancy of solid uterine tumors. CONCLUSION: Uterine artery embolization should be the last resort for the treatment of symptomatic fibroids. [ABSTRACT FROM AUTHOR] |
