TURKISH ARMED FORCES RESPONSE AGAINST MEASLES OUTBREAK IN MILITARY UNITS.

Abstrakt: OBJECTIVE: We investigate the kinetics of an outbreak of measles in the military units MATERIAL AND METHOD: Soldiers diagnosed as measles not only with clinical findings such as rash, koplik‟s spot, pharyngeal hyperemia and crude lung sounds, but also with the presence of IgM antibodies by ELISA for rubeola in the reference laboratory of the Public Health Agency of Turkey. Also, suspected cases were diagnosed by PCR testing. Age distribution of measles cases were examined by the Scientific Advisory Board of Turkish Ministry of Health. They decided that recruited privates born in 1980 -1991 should be immunized with a single dose of MMR vaccine. RESULTS: Totally 204 cases were diagnosed in Turkish Armed Forces (TAF) in the winter season of 2012-2013. Of all the TAF cases, which were reported to the national surveillance system, 83 (% 40,68) symptoms were quite obvious and remained moderate. There were no deaths or encephalitis. Four (%2) hospitalized patients showed severe respiratory symptoms (dyspnea, cough). 35 patients (%17) had elevated transaminase levels (200-400 IU/L). Nearly 200.000 military personnel were vaccinated with the single dose of MMR in three months period. CONCLUSION: Soldiers are important for the course of adult measles. Despite the eradication programs, occurrence of measles outbreak showed us that the desired immunization rates could not be reached. Outbreaks in communal areas for the adult age group vaccination programs should closely be monitored. The development of new measles vaccination strategies is necessary for the prevention of rapid spreading diseases such as measles. [ABSTRACT FROM AUTHOR]
