Abstrakt: |
Nowadays, a problem of great topical interest for Health Systems is the care of frail people. These individuals require both health and social care services (integrated care), preferably provided at home. From this viewpoint, both the literature and empirical experience suggest the necessity for new proposals for ICT architectures that can cost-effectively integrate processes without introducing significant changes to organizational models (which are often subject to legal constraints). These architectures need be properly supported by process (re)engineering, and need to consider features such as: sustainable development and maintenance, enhancement of existing technologies and focus on integration between care providers. The integration of smart domotic tools completes the picture, including patient's homes in care processes. Issues such as end-to-end services and macro-classes of patients have to be taken into account when developing such integrated systems. Furthermore, the solutions should be replicable in other districts, while respecting the various specificities. This paper describes the solutions being developed for a project in Italy, whose purpose is to define architectures and systems that are coherent with the above features that will be piloted in Lombardy.
