Aim: To compare the Marshall Score with the APACHE II score in assessing the severity of acute pancreatitis. Methods and Material: Sixty-two cases of suspected acute pancreatitis admitted to the Department of Surgery, PGIMER & DR RML Hospital were assessed using Marshall and APACHE II scores at admission, and thereafter at 48-hour intervals till discharge or death of the patient. Results: This study demonstrated that the APACHE II scoring system is better than the Marshall scoring system in predicting the severity of acute pancreatitis. The sensitivity of the Marshall Score was 54.2%, specificity 100%, PPV 100%, NPV 70.3% and accuracy was 78.0% on day 1, which was less comparable with APACHE II as per the Atlanta classification. The Marshall Score showed a decreasing trend of sensitivity, NPV and accuracy for days 3, 5 and 7. Conclusion: This study demonstrated that the Marshall scoring system is not reliable in predicting the severity of acute pancreatitis when compared to the APACHE II scoring system.
