Anterior resection and “pull-through” procedures for cancer of the rectum and rectosigmoid.

Abstrakt: Cancer of the lower level of the rectum requires removal of the rectum and establishment of a permanent abdominal colonic stoma as described by Miles. It is feasible technically to treat most cancers of the midrectum and the upper portion of the rectum by either anterior resection or a pull-through operation which preserves the anus and some or all of the sphincteric apparatus. The wisdom of such a course will be decided by a comparison of results of the various operations. A substantial amount of data, reviewed in this paper, indicate that five-year survival rates and operative mortality rates after the sphincter-preserving operations can compare favorably with those for similar lesions treated by the Miles operation. These data encourage my colleagues and me to continue use of anterior resection and pull-through procedures for many cancers of the midrectum and upper rectum. [ABSTRACT FROM AUTHOR]
