Abstrakt: Approximately 45.8 million Americans smoke. Smoking is a leading cause of preventable disease, disability, and death. The Wellness Division of VNA Care Plus, recognizing the relationship between tobacco use and disease, now empowers smokers to progress toward cessation. It has adapted the National Cancer Institute Smoking Cessation Model for Primary Care Sites. The agency's assessment tool and benefits and barriers survey affecting tobacco use are included. The profile of smokers and 'Tips for Treating Elder Smokers' are described. Recommendations include standardized protocol, consistent interventions from providers, and suggestions for further study.
