New curricula designed to integrate basic sciences with clinical medicine have been instituted recently at medical schools across the United States. One evaluation approach that is now widely used to assess students' clinical skills is the Objective Structured Clinical Examination (OSCE). The OSCE measures clinical competence in a structured, multi-stationed, objective fashion, often using standardized patients (SP). Most OSCE efforts measure clinical skills independent from basic science knowledge at separate stations in the examination. To date a measurement tool has not been documented that measures the integration of scientific knowledge and clinical communication skills in the standardized patient methodology. The article describes the development of an assessment tool to measure students' ability to integrate and apply their basic science knowledge during the clinical exam. Since this new tool measures integration using standardized patients, the instrument will be termed the I-OSCE. The State University of New York at Buffalo School of Medicine and Biomedical Sciences in New York State instituted a new systems-based curriculum for first- and second-year medical students in fall 2001. The new curriculum emphasizes integration of basic and clinical sciences in the framework of patient-centered, case-based learning. Assessment of students' clinical skill acquisition was evaluated in a traditional OSCE format that included both interviews and physical examination with SP at the end of each semester. In-room faculty evaluated students using a traditional binary checklist and overall evaluation of the encounter by means of a four-point Likert scale. The I-OSCE merits attention for several reasons. It also represents a first effort to assess students' demonstrated ability to integrate their basic science knowledge and their clinical communication skills. |
