Brief Background: Restoring the form, function and aesthetics of a patient is as important as extracting the non-salvageable carious teeth in order to improve their oral health related quality of life.A holistic approach is necessary not only to improve the oral condition but also for the overall well-being of the patient. Described here is a case of Severe Early Childhood Caries (S-ECC) in a four year old child and its management. Materials and Methods: After examination and diagnosis, with due consideration to behaviour modification, treatment was performed which included restorations and extractions of carious teeth. In order to preserve the space and restore the form, function and aesthetics, a modified fixed semifunctional appliance (Pedopartial) was fabricated and delivered. Discussions: There are many appliances that can be considered postextraction of primary teeth in children. Apart from satisfying the needs of the patient in restoring aesthetics, the Pedo-partial appliance (modified Nance Appliance) used in this case aided in preventing problems like speech alteration, development of deleterious habits, space maintenance and most importantly the psychological trauma related to lost teeth. Careful fabrication of the appliance and patient compliance for the maintenance and follow-ups were the keys to the successful outcome. Summary and Conclusions: The importance of restoring the quality of life in a child suffering from the consequences of ECC need not be overemphasized. Pedo-partial appliancecan prove to be an economical and functionally viable treatment of choice for a child who expresses aesthetic concern. However, careful fabrication, good compliance on the part of the patient and regular preventive care and follow-up are necessary for its success. [ABSTRACT FROM AUTHOR] |
