Expansive periapical lesions often lead the clinician toward surgical interventions, to facilitate faster resolution of the pathology. Innumerable materials at our disposal, when judiciously applied in conservative nonsurgical interventions, present excellent results. Detailed here is such a case, with a large periapical lesion i.r.t. maxillary left central incisor and lateral incisor, resolved in a relatively short timeframe with the unusual combination of calcium hydroxide chlorehexidine paste.Significant bone formation was seen at the periapical region at the 1 year follow-up.
