Abstrakt: Competing Interests: DD declares being the non-remunerated, non-executive Board Director for the International Society for Pharmacoeconomics and Outcomes Research (ISPOR). DD and JE are employees of National Institute for Health and Care Excellence, the national health technology assessment organisation for England.
