[Collagenase-A revolution of Dupuytren's therapy].

Abstrakt: Up to date, Dupuytren's contracture is a disease without cure. Almost 20 years ago, enzymatic cord degradation with clostridial collagenase was described for the first time. Whereas until recently limited fasciectomy was the treatment of choice for most of the patients, now collagenase offers a minimally invasive treatment in cases with palpable cords. Collagenase is a safe and effective treatment with success rates around 85 % for MCP joint and 65 % for PIP joints and a low complication rate. One point of concern is the recurrence rate which is higher compared to open surgery. A review of the literature is presented, technique and indication for collagenase are discussed.
