Abstrakt: A 33-year-old man visited the outpatient department of the authors of this article with a large perineal mass. The patient was an active amateur racing cyclist. At physical examination the swelling resembled an accessory scrotum. Ultrasonography revealed a subcutaneous thickening but no intrascrotal abnormalities. Because of progressive symptoms the perineal tumour was surgically excised. Histopathologic examination revealed cell deficient and degenerative connective tissue. The tissue possibly contained a bursa. The tumour was diagnosed as perineal nodular induration, a rare lesion. These nodules almost invariably affect keen cyclists after sustained cycling. Recognition of perineal nodular induration is of particular interest to general practitioners, urologists and specialists in sports medicine.
