A study was made of the glomerular filtration rate (GFR) under medicamentous loading with indometacin in 60 patients with insulin-dependent diabetes mellitus without apparent clinical signs of diabetic nephropathy (DN) with the purpose of finding ways of applying the given method for early diagnosis of subclinical stages of nephropathy. 49 patients revealed II to III stage DN as to Mogensen C. E. classification, characterized by hyperfiltration and microalbuminuria. Lack of adequate response to intake of indometacin manifested by an increase in the initially high GFR was recordable in 66% of patients with stage II DN and in 56% of those with stage II DN. Thus, indometacin test can be recommended for use as an additional method of early diagnosis of preclinical stage DN in patients with increased GFR. |
