The attribution of care responsibilities of return visits for institutional long-term residents under Taiwan NHI

Background: The rate of “Return visit to emergency department of the same hospital within three days after discharge ”is used to show whether the medical care of the people during hospitalization is properly taken care;so it is regarded as an important indicator for judging the quality of care in this hospital. However, in the remote rural areas, district hospitals or small regional hospitals that take care of long-term care institution residents and local old people, due to patient’s composition, this index is likely to be higher than the “divisional index value”. The degree of punishment is unfair and not objective. Objective: In this study, it is hoped that we can explore if the indicator of “Return visit to emergency department of the same hospital within three days after discharge” in the patients of long-term care institutions is significantly higher than the one of ordinary patients? Are there differences between different long-term care institutions and nursing homes? Are there differences between long-term care institutions with different number of beds? Methods: In this study, I would like to take a case hospital as an example. According to identity, the patients discharged from the hospital from 2014 to 2016are divided into long-term care institution patients and ordinary patients. We analyze the rate of “Return visit to emergency department of the same hospital within three days after discharge” in patients from 17 long-term care institutions cooperating with the hospital land ordinary patients. The ratios are compared and analyzed. Results: The indicators of “Return visit to emergency department of the same hospital within three days after discharge ”in long-term care institution patients were significantly higher than those of ordinary patients;and there was no significant difference between the long-term care institutions and nursing homes;there was also no significant difference between the long-term care institutions with different scales. Conclusion: It is not easy to survive in district hospitals or small regional hospitals that take care of long-term care institution residents and local old people in remote areas; it should no longer be criticized or punished in a manner of formal equality; this may make the indicator become an ineffective indicator, also could not see the real problems!
