A National Survey on Occupational Exposure, Health Status, and Intention to Stay among Nurse Anesthetists

Background: Anesthesia is indispensable during surgeries and most of invasive procedures. Nurse anesthetists (NAs) are important members of anesthesia care team. They need to be vigilant in all the course of anesthesia to discover and avoid danger that may jeopardize clients’ health and life. Nurse anesthetists are exposed to multiple occupational exposures that may harm their health and even affect their willing to stay in their current job and profession. The health of nurse anesthetists may affect the quality of anesthesia care and patient safety. The retention of nurse anesthetists is necessary to maintain the adequate anesthetic care. There are quite short of enough knowledge about the occupational exposure, health status, and the intention to stay of nurse anesthetists. Method: A cross-sectional survey with self-administered questionnaires was conducted. The samples were members of the Taiwan association of nurse anesthetists. A total of 1532 questionnaires were mailed to 149 hospitals and clinics on September 4th, 2013. After 42days, 1430 questionnaires were returned with 1395 completely filled. The completion rate was 91% (1395/1532). The questionnaire included social-demographics, health behavior, job characteristics, occupational exposures, health status, and intention to stay. Data were processed and analyzed with PASW statistics 18.0 for Windows. Descriptive statistics, spearman correlation, logistic regression, and general linear model were applied to perform data analysis. Result: The results showed that the mean age of NAs was 37 (SD=1.56) years old, 99% were female, and the mean work years as NAs were13.39 (SD=8.45) years. The prevalences of occupational exposures were psychosocial exposure-work pressure (99.5%), physical exposure-rotating shifts (87.81), frequently work overtime (84.01%), biologic exposure (86.52%),chemical exposure (85.23%), environmental exposure-radiation without protective equipment (83.34%). The prevalences of health problem were abnormal menstruation(14.09%), menstrual syndrome (12.15%), frequently flu (10.17%), uterine myoma (8.31%), mayofasciitis over neck and shoulder (7.77%), urinary tract infections (5.88%), varicose vein(5.53%), atopic dermatitis (5.41%), lumbar herniated intervertebral disc (5.30%)、miscarriage(5.22%), and occupational allergy (5.10%). The health quality of life as measured by SF-36 was 51.45 (SD=8.03) for physical components (PCS) and 36.40(±12.47) for mental components (MCS). There were 31.98% of NAs planned to leave their job as NAs. Those who reported higher level of work being affordable had higher PCS and MCS. The more occupational exposures were associated with the worse health status and the lower intention to stay. Conclusions: Similar to general nurses, genitourinary system problem had the highest prevalence in NAs. But the immunity system and skeletal muscle had higher prevalence in NAs than in nurses. The health quality of life of NAs were worse than national norm. Work characteristics and occupational exposures affected the health status and intention to stay among NAs. Strategies should be developed and applied to improve the work condition and prevent occupational exposures in order to promote health and retention of NAs.
