Symptom Distress and Home Care Needs in Oral Cancer Patients

The major treatment for oral cancer patients is surgery, supplemented by radiotherapy and chemotherapy. In addition to the disease itself, many patients feel uncomfortable due to the process of medical treatment and complications of these treatments. Those problems will be accompanied patients in the rest of their lives. The main purpose of this study aimed to investigate the uncomfortable situations of the patients with oral cancer in the physical, psychological and social functions and home care needs after they went through the treatment of surgery, radiotherapy and chemotherapy. This was a cross-sectional survey study. The samples of this study were 97 patients who were diagnosed oral cancer more than six months and had received surgical treatment, radiotherapy, and chemotherapy in a medical center in Taipei. The data were collected by the structured questionnaire made by the researcher. The questionnaire contains three parts: demographic data, physical, psychological, and social symptom distress scale, home care needs scale. The data were analyzed by descriptive statistics, t-test, one-way ANOVA, Pearson correlation method for statistical analysis. The results showed that: the distress of physical symptoms in oral cancer patients was most troubled in "neck problems", "feeding problems", "speech problems"; the distress of psychological symptoms was most troubled in "the joy of living", "the future expectations", and "emotional problems"; finally, the distress of social symptoms was most troubled in "social interaction problems", "life issues", and "relationships with others". The highest demands of home care needs in oral cancer patients were in "information needs", "physiological needs", and "social needs". The distress of physical, psychological, social symptoms and home care needs were positively correlated with each other. Namely, the more distress in physical symptom is, the more psychological symptom and serious social symptom, and demands for home care will be. From this study, we can know each distress of oral cancer patients in physical, psychological, and social symptoms, as well as exact needs in their home life. This study suggests that the clinical staffs understand the problem the patients may face, and provides more care information, prevention and early intervention in rehabilitation and psychological development for the distress of those symptoms to assist patients in adapting a new life after the treatment.
