Although school-based fluoride mouth-rinsing program is one of the most effective and widely used approach for caries prevention and control, some controversies about its application on pre-school children were found in the literatures for the under development of swallowing mechanism. The purposes of this study were to determine the change of urinary fluoride concentration and the affecting factors after sodium fluoride mouth-rinse by preschool children. Five kindergartens from three districts in Taipei city were selected on account of good compliance. Case-control study design was used. Fifty eight 5 year-old children, who rinsed with 10 c.c. of 0.05% sodium fluoride mouth-rinse for one minute once per week at school under the supervision of their teachers, were in the test group. After matched with sex, fifty seven 4 year-old children in the same kindergarten were selected as negative control group. The rinse program started from December 2002 to July 2003. Three urine samples, from first spot-urine, after rinsing for 2 hours, and for 4 hours each, were collected in sealed plastic containers on the assigned date, which were the day before the program stated, the day of the 3rd, 7th, 11th,15th, and 23th week of rinsing program. Urinary fluoride concentration was determined with a fluoride ion specific electrode(Orion Model 96-6). All data were processed with frequency, percentile, t-test, one—way ANOVA and paired—t test by SPSS 10.0 for windows / pc software. The mean urinary fluoride concentration in test group were 0.482 ppm for the first spot-urine, and 0.451, 0.491 ppm for the 2 hours and 4 hours samples, while in control group the mean urinary fluoride were 0.461, 0.416, 0.423 ppm respectively. No statistically significant difference of age, gender, school location were found between test and control groups (p
