A new surgical approach for managing iatrogenic urinary obstruction after midurethral sling surgery

Popis: Background: A common complication of anti-incontinence procedures is iatrogenic bladder outlet obstruction (BOO). Various methods for performing a sling incision have been described, however, the optimal management of BOO after sling placement remains controversial. Objective: To describe a novel in-situ bilateral sling incision technique for BOO and to compare its long-term outcomes with those of the traditional midline sling cut. Materials and methods: A cross-sectional study of outcomes of 63 female patients who underwent two types of sling cut surgery for BOO was performed. Symptom resolution and the presence of stress urinary incontinence symptoms after a sling incision were assessed by phone interview at least 1 year after surgery. Results: The study group (n = 16) was comparable with the controls (n = 47) with respect to patient reports of presenting symptom resolution, but not to recurrent SUI symptom frequency. This was threefold lower in the study group. Conclusions: This video shows that the novel bilateral in-situ suburethral sling cut technique is an effective, minimally invasive therapeutic option for selected patients with iatrogenic urinary obstruction after MUS surgery. It gave comparable functional results to those of traditional mid-line incision/excision surgery while maintaining urinary continence in a majority of cases.
