Popis: | A busca por procedimentos minimamente invasivos para o rejuvenescimento e remodela??o da face tem sido crescente nos ?ltimos anos. O uso do Polimetilmetacrilato (PMMA) representa uma das op??es e ganhou muita popularidade. Este pol?mero ? composto por microesferas de di?metro vari?vel e dispersas em algum ve?culo como col?geno bovino, polietilenoglicol ou ?cido hialur?nico, sendo considerado um produto de preenchimento permanente. Apesar da aparente simplicidade do m?todo de aplica??o, as complica??es relacionadas a este procedimento, principalmente na face, tamb?m t?m crescido e s?o muitas vezes, graves, imprevis?veis quanto a seu aparecimento, momento de surgimento ou regi?o anat?mica comprometida. O objetivo deste estudo foi avaliar retrospetivamente o tempo mediano de ocorr?ncia das complica??es, regi?es mais frequentes de ocorr?ncia, tipo de manifesta??o e seus tratamentos pr?vios. Al?m disso, buscou esclarecer se o n?mero de vezes em que o PMMA foi injetado apresentou rela??o com tempo de surgimento mais precoce das complica??es e determinar se havia diferen?a quanto ao tempo de ocorr?ncia de complica??es segundo o sexo e idade. Foi realizado estudo retrospectivo descritivo de casos de uma popula??o restrita (observacional) que buscou tratamento em cl?nica pessoal no per?odo de janeiro de 2000 a junho de 2021. Atrav?s de informa??es obtidas do prontu?rio eletr?nico, foram arrolados 209 indiv?duos portadores de complica??es j? estabelecidas e relacionadas ? aplica??o pr?via de PMMA na face realizada por outros profissionais m?dicos. As complica??es foram classificadas segundo informa??es obtidas a partir de hist?ria e exame cl?nico, exames de imagem, anatomopatol?gico e/ou an?lise fotogr?fica. As seguintes vari?veis foram descritas e analisadas: sexo, idade, quantidade de vezes que foi aplicado o material, tempo de aparecimento da complica??o, tipos de complica??es e suas localiza??es, ?rea injetada que mais complicou e procedimentos previamente realizados por outros profissionais visando o tratamento das complica??es. Na descri??o de ocorr?ncia dos eventos avaliados ao longo do tempo, a an?lise estat?stica se baseou na elabora??o de curvas de Kaplan-Meier. A compara??o entre os grupos estudados foi realizada atrav?s de teste de log-rank. Valores de p < 0,05 foram considerados estatisticamente significativos. A m?dia de idade foi de 44,6 anos com desvio-padr?o 12,2 anos, 172 eram mulheres e 37 homens. A idade de maior incid?ncia de casos foi de 35 anos. A an?lise de dados mostrou um tempo mediano de surgimento de complica??es de 71 meses ap?s a inje??o inicial de PMMA na face. A complica??o mais precoce ocorreu apenas um m?s ap?s a aplica??o deste produto e a mais tardia 330 meses. As regi?es malar (48,8%) e mandibular (47,8%) foram as ?reas mais comprometidas, seguidas pela regi?o zigom?tica (43,5%), mento (27,8%), l?bios (27,3%) e sulcos nasogeniano (22%). O nariz (10%), p?lpebras (8,6%), glabela (8,6%), ?rea temporal (7,2%), frontal (1,9%) e orelhas (1%) tamb?m foram comprometidas com complica??es, por?m em menor frequ?ncia. A complica??o mais frequente foi granuloma presente em 64,6% dos pacientes, seguido pelo edema (57,4%) e inflama??o (37,3%). Altera??es funcionais, forma??o de n?dulos, neovasos, altera??es de pigmenta??o cut?nea, lacrimejamento, infec??o, necrose e forma??o de f?stulas tamb?m foram encontrados no estudo. O tratamento mais realizado foi a inje??o local de corticoide em 53,1% dos pacientes, seguido pela remo??o cir?rgica (19,1%). Outros recursos utilizados na tentativa de tratamento das complica??es foram a inje??o intra-lesional de 5- fluoruracil, uso de ozonioterapia, aspira??o do local comprometido, bichectomia, uso de xilitol, alopurinol e remo??o com laser intra-lesional. Na popula??o avaliada, n?o houve rela??o entre a vari?vel n?mero de vezes em que o PMMA foi injetado e o surgimento precoce de complica??es (p = 0,73). N?o foi encontrada diferen?a significativa entre homens e mulheres no tempo de ocorr?ncia de complica??es (p = 0,27). Na compara??o entre grupos atrav?s do teste de log-rank, o valor de p < 0,001 evidenciou diferen?a estatisticamente significativa, concluindo que pacientes com idade menor de 50 anos apresentaram surgimento de complica??es mais precoce que aqueles com idade maior de 50 anos. Conclui-se que as complica??es relacionadas ao uso de PMMA podem ocorrer muito tempo ap?s sua inje??o. O estudo demonstrou tempo mediano de 71 meses e com complica??es tardias ocorrendo mais de 25 anos ap?s sua aplica??o na face. Portanto, o acompanhamento dos pacientes submetidos a este tratamento, orienta??o sobre riscos, caracter?sticas do material, complica??es e manejo adequado s?o de fundamental import?ncia. Apesar das regi?es malar, mandibular e zigom?tica terem sido as mais comprometidas, todas as ?reas da face apresentaram algum tipo de complica??o. O diagn?stico preciso, auxiliado muitas vezes por exames de imagem ou histol?gico, auxilia no diagn?stico diferencial entre n?dulos e granulomas, orientando um tratamento mais espec?fico. Quanto ao fato de a quantidade de vezes em que o PMMA foi injetado n?o ter apresentado rela??o com o surgimento de complica??es, possivelmente o volume total injetado e/ou sua qualidade, ainda que n?o avaliados, possam apresentar rela??o com as complica??es. Os resultados podem servir de base de conhecimento, auxiliando numa melhor compreens?o das complica??es relacionadas ao uso de PMMA na face. The search for minimally invasive procedures for facial rejuvenation and remodeling has been increasing in recent years. The use of polymethylmethacrylate (PMMA) represents one of the options that has gained a lot of popularity. This polymer is composed of variable- sized PMMA microspheres that are dispersed in some vehicle such as bovine collagen, polyethylene glycol or hyaluronic acid. It is considered a permanent filler product. Despite the apparent simplicity in the method of application, complications related to this procedure, especially in the face, have increased and are often severe, unpredictable as to the time of onset or anatomical region involved. This study sought to retrospectively assess important points in this population and clarify the median time of occurrence of complications, the most frequent regions of occurrence, the type of clinical manifestation and any previous treatments. In addition, we sought to clarify whether there was a relation between the number of times PMMA was injected and an earlier time of complication onset and to determine whether there was a difference in the occurrence of complications according to sex and age. A retrospective descriptive study of cases from a restricted (observational) population that sought treatment in the author?s private clinic from January 2000 to June 2021 was carried out. Through information obtained from the electronic medical record, 209 individuals with complications related to previous injection of polymethylmethacrylate (PMMA) in the face, performed by other medical professionals, were enrolled. Complications were classified according to information obtained from the history and clinical examination, imaging examination, anatomopathological study and/or photographic analysis. The following variables were described and analyzed: age, sex, number of times PMMA was injected, type of complication and its anatomic location, time of onset of the complication, procedures previously performed by other professionals to treat the complications and the anatomic area most frequently involved in complications. In describing the occurrence of events evaluated over time, the statistical analysis was based on the development of Kaplan-Meier curves. The comparison between the studied groups was performed using the log-rank test. Values of p < 0.05 were considered statistically significant. The mean age was 44.6 years with a standard deviation of 12.2 years; 172 were women and 37 were men. The age of highest incidence of cases was 35 years. Data analysis showed a median time to onset of complications of 71 months after the initial injection of PMMA in the face. The earliest complication occurred just one month after injection of this product and the latest at 330 months. The malar (48.8%) and mandibular (47.8%) regions were the most affected areas, followed by the zygomatic (43.5%) region, mentum (27.8%), lips (27.3%) and nasolabial folds (22.0%). The nose (10%), eyelids (8.6%), glabella (8.6%), temporal area (7.2%), forehead (1.9%) and ears (1.0%) were also involved with complications, but less frequently. The most frequent complication was granuloma, present in 64.6% of patients, followed by edema (57.4%) and inflammation (37.3%). Functional alterations, formation of nodules, neovessels, alterations in skin pigmentation, tearing, infection, necrosis and fistula formation were also found in the study. The most common treatment was local injection of corticosteroids in 53.1% of patients, followed by surgical removal in 19.1%. Other resources used in an attempt to treat complications were intralesional injection of 5-fluorouracil, use of ozone therapy, aspiration of the compromised site, Bichectomy, use of xylitol, allopurinol and intralesional laser removal. In the population evaluated, there was no relationship between the number of times PMMA was injected and the time of onset of complications (p = 0.73). In the comparison between groups using the log-rank test, the value of p < 0.001 showed a statistically significant difference. Patients under 50 years of age presented complications earlier than those over 50 years of age. Complications related to the use of PMMA can occur long time after its injection. The study showed a median time of 71 months and late complications occurring more than 25 years after its application to the face. Therefore, the follow-up of patients undergoing this treatment, guidance on risks, material characteristics, complications and proper management are of fundamental importance. Although the malar, mandibular and zygomatic regions were the most affected, all areas of the face presented some type of complication. Accurate diagnosis, often aided by imaging examination or histological tests, helps in the differential diagnosis between nodules and granulomas, guiding a more specific treatment. The fact that the number of times PMMA was injected was not related to the emergence of complications, possibly the total volume injected and/or its quality, even if not evaluated, may be related to complications.
