Nailfold capillaroscopy in paediatrics – single centre experience

Popis: Cilj ovog rada je prikazati metodu kapilaroskopije, način izvođenja i indikacije za upućivanje pacijenata uz pregled dosadašnjih iskustava Odjela za reumatologiju Dječje bolnice Srebrnjak. Retrospektivno smo pretražili bolnički informatički sustav Dječje bolnice Srebrnjak i formirali bazu bolesnika kojima je učinjena kapilaroskopija te analizirali dijagnoze zbog kojih je pretraga bila indicirana, nađene kapilaroskopske promjene kao i dobnu i spolnu strukturu uključenih bolesnika. U razdoblju od 1.1.2016. - 31.12.2018. obavljeno je 465 kapilaroskopskih pregleda ležišta noktiju kod ukupno 127-ero pacijenata (102 djevojčice, 80,3%, i 25 dječaka, 19,7%). Prosječna dob u vrijeme izvođenja pretrage bila je 14,9 godina. Kod većine bolesnika kapilaroskopski pregled je indicirao pedijatrijski reumatolog (99-ero bolesnika), a ostale pedijatri drugih supspecijalnosti (pulmolozi, neurolozi), a jedan je pregled indicirao infektolog. Od ukupno 465 obavljenih pregleda uredan nalaz je nađen u 5,8% slučajeva, a kod ostalih bolesnika su nađene promjene mikrovaskulature baze nokta. Kapilaroskopija je neinvazivna, jednostavna i lakoponovljiva dijagnostička metoda, koja se pokazala ključnom za postavljanje dijagnoze primarnog Raynaudovog sindroma te nezaobilaznom pretragom u sklopu dijagnostičke obrade sekundarnog Raynaudovog sindroma i drugih reumatskih bolesti (sklerodermija, vaskulitisi, miješana bolest vezivnog tkiva, dermatomiozitis, juvenilni idiopatski artritis...). Nadamo se da će se ovaj prikaz približiti metodu ostalim supspecijalističkim strukama koje obrađuju i liječe bolesnika s očekivanim promjenama na mikrovaskulaturi te da će se metoda početi indicirati i u dosad manje zastupljenim dijagnozama.
The aim of this paper is to present nailfold capillaroscopy, explain the steps while performing the procedure and indications for capillaroscopy combined with presentation of current experience with the method at the Department of Rheumatology, Srebrnjak Children’s Hospital. Retrospective search of hospital patient records was performed in order to identify patients who underwent capillaroscopy. We analysed the indications for the procedure, capillaroscopic findings, and gender and age distribution of patients. During the period from January 1st 2016 until December 31st 2018, we performed 465 nailfold capillaroscopies in 127 patients (102 (80.3%) girls and 25 (19.7%) boys), mean age 14.9 years. Indication for nailfold capillaroscopy was made by paediatric rheumatologist in 99 patients, and the rest of the procedures were recommended by other paediatric subspecialists (pulmonologist, neurologist and infectious disease specialist). Normal capillaroscopy finding was recorded in 27 (5.8%) patients, whereas the remaining patients had evident microvascular abnormalities. Nailfold capillaroscopy is a non-invasive, simple, easily reproducible procedure crucial for diagnosing primary Raynaud syndrome, but also part of diagnostic workup for secondary Raynaud syndrome and other rheumatic disease (scleroderma, vasculitis, mixed connective tissue disease, dermatomyositis and juvenile idiopathic arthritis). We hope that this paper will familiarise other paediatric subspecialists involved in the diagnosis and treatment of the diseases causing microvascular abnormalities with this method and lead to the introduction of capillaroscopy in their practice
