Septic shock: hemodynamics of endotoxin and inflammation

Popis: To evaluate the relations between bacterial endotoxin and the active hyperemia of inflammation, observations have been made in fifty-three mongrel dogs in which a nonseptic inflammatory lesion was induced by the intramuscular injection of 10 per cent calcium chloride into one thigh. Observations made four days later have demonstrated that the inflammatory lesion was associated with an increase in blood flow (91 per cent greater than that in the normal limb) and a corresponding reduction in vascular resistance in the limb. The arteriovenous oxygen difference was decreased, and the calculated oxygen consumption was 26 per cent greater than that in the normal limb. The administration of endotoxin ( E coli , 0.2 mg/kg) was associated with a marked reduction in blood flow and an increase in vascular resistance in both inflamed and normal limbs. These changes were not greatly altered by the prior administration of phenoxybenzamine (1.0 mg/kg) or methylprednisolone (30 mg/kg). Expansion of the circulating blood volume with dextran −70 solution was followed by a marked increase in flow and reduction in vascular resistance in the normal and inflamed limbs. It is concluded that the active hyperemia of inflammation may add significantly to demands on the circulation. The hyperdynamic state is reduced after the administration of endotoxin and returns when the circulating blood volume is increased above normal by the infusion of dextran solution.
