Popis: | The present work deals with the development of an Accident Precursors Management System, aimed at analysing the accident precursors (unsafe act & conditions and near misses) in order to identify the preventive actions needed to avoid them and the accidents that could descend from the analysed events. The methodology is developed starting from the HFACS taxonomy for the collection and classification of the observed events and the Fuzzy Application Procedure – FAP, already devised for the occupational accident risk analysis in the industrial domain. The methodology proposed is thus made of: a data collection procedure, to be carried out in situ based on the observation of the workshop, and a data analysis tool, based on the Fuzzy Logic Approach, that allows to obtain the suitable preventive measures to cope with the accident precursors analysed. The methodology described is generally applicable and does not depend on a specific working site environment. On the contrary, the measures to be applied to prevent accident precursors and their effectiveness are strongly dependent on the work environment, the safety culture and the operational procedures and praxis. The method has been tested in a real industrial workplace and the results obtained are here described and discussed. |
