Normal and Post-Exercise Plasma Transfusion in Patients with Haemophilia A and von Willebrand's Disease

Popis: Normal and post-exercise plasma from the same donor was transfused into seven patients (into one patient twice) with haemophilia A and two patients with von Willebrand's disease. After post-exercise plasma administration, on the average twice as much factor VIII activity was recovered in the patients than after normal plasma. Factor VIII levels after post-exercise plasma transfusions, however, were clearly lower than the expected levels calculated from the in vitro activity of the transfused plasmas, especially when recalcification assay was used. Too high factor VIII values of post-exercise plasmas in this assay system were due to some activation products, which clearly had less effect on the results of cephalin-kaolin assay. The better in vivo effect of post-exercise plasmas is regarded as an additional proof of real factor VIII increase during muscular exercise and the possibilities for the therapeutic use of post-exercise plasmas are shortly discussed. In two patients with von Willebrand's disease the primary factor VIII response was comparable with that of A-haemophiliacs and the bleeding time response better after post-exercise plasmas, but no difference was observed in the secondary factor VIII response.
