Oral contraceptives, anti-thrombin III and fibrinolytic activity in Africans

Popis: A comparison of the effects of progestogen-only and combined progestogen-oestrogen types of oral contraceptives on anti-thrombin III activity and fibrinolysis in African subjects in South Africa is reported. The changes in anti-thrombin III activity are similar to those reported in other races, but the augmentation of fibrinolytic activity is more marked than has been reported elsewhere. The possible significance of these findings is discussed.Oral contraceptives were used by African women both with the combination type (norgestrel .5 mg and ethinyl estradiol .05 mg) and with the progestogen-only type (noresthisterone .35 mg); controls received no medication. Serum antithrombin 3 was measured, and for fibrinolytic activity the euglobulin lysis time was determined. The fibrin plate assay was used. A highly significant reduction in the level of antithrombin 3 activity was found in those receiving the combination oral contraceptive. Those receiving the progestogen-only type showed some increase in the antithrombin 3 levels. A highly significant increase in fibrinolytic activity was observed in those receiving the combination drug but no significant change in those receiving the progestogen only. These changes were similar to those reported for other races elsewhere, but the increase in fibrinolytic activity was more marked. There may be some advantage in using progestogen-only contraceptives in women with a high risk of thrombosis.
