A Comparative Evaluation of Ftorafur and 5-Fluorouracil within Combination Chemotherapy (HexaCAF vs. HexaCAFt) of Ovarian Cancer

Popis: SummaryA controlled randomized clinical study was carried out in ovarian carcinoma stage 3–4 (FIGO) comparing two regimens, HexaCAF (hexamethylmelamine 150 mg/m2 day 1–14, cyclophosphamide 150 mg/m2 per os day 1–14, methotrexate 40 mg/m2 i. v. day 1 and 8, 5-fluorouracil 600 mg/m2 i. v. day 1 and 8) and HexaCAFt (the same regimen with 5-flourouracil substituted by Ftorafur 600 mg/m2 i. v. day 1–14). Treatment cycles were repeated with intervals of 2–3 weeks up to progression, in responders up to six cycles and succeeding chemotherapy up to one year.HexaCAF used in 29 patients resulted in 10 (34%) complete responses (CR) and 11 (38%) partial responses (PR). HexaCAFt used in 25 patients produced CR in 8 (32%) and PR in 9 (36%) cases. Mean duration of remission with HexaCAF was 17 months, with HexaCAFt 15 months. Gastrointestinal and heamatologic toxicity were similar in both treatment regimens, but HexaCAF required more dose limitations during the course of treatment than HexaCAFt because of increasing haematotoxicity. Thus, substitution of 5-fluorouracil by Ftorafur in the combination studied did not improve the antitumor activity but lowered its toxicity.
