Volumetric capnography: a promising lung function test in cystic fibrosis?

Popis: | Introduction: Volumetric Capnography (VCap) is a quick and easy method to assess ventilation inhomogeneity (VI). It requires only few tidal breaths and no extensive data processing. Thus, VCap omits challenges that currently limit the use of established VI-tests, such as Multiple Breath Washout (MBW). Aim: We assessed VCap feasibility, reproducibility and comparability with the Lung Clearance Index (LCI) of Cystic Fibrosis (CF) N2-MBW data. Methods: We included 317 N2-MBW trials from 61 healthy and 54 CF children (age 9.5±4.1 and 10.1±3.9 years) with at least two good quality MBW trials. Primary outcomes were LCI and capnographic inhomogeneity indices (CII). CII were calculated as the relative end-tidal CO2 concentration differences (%) between the air in the dead space and the total alveolar air (CII1), expired alveolar air (CII2) and alveolar air in the conductive airways (CII3), respectively. Results: CII calculation was feasible in 100% of the trials. LCI and CII1-3 were increased in CF compared to healthy children (LCI 9.2±2.3 vs 7.1±0.6, p Conclusion: CII calculation is feasible in good quality MBW trials. CII correlate with LCI, but show slightly higher variability. VCap is thus a fast and promising VI-test with high potential for clinical use. Further studies on feasibility and methodology to define minimal data quality criteria seem thus justified. |
