The first line of therapy for breathing-related sleep disorders remains positive airway pressure (PAP). These disorders can fall into one or more of three categories: obstructive sleep apnea (OSA), central sleep apnea (CSA), and hypoventilation. Treatment of OSA involves opening the airway during sleep with either PAP, oral appliance therapy, or behavioral modification. Treatment of CSA requires the initiation and support of inspiratory effort when a patient’s own respiratory drive is either absent or insufficient. Hypoventilation requires support and augmentation of ventilation with noninvasive positive pressure ventilator devices. Treatment of these disorders always includes longitudinal clinical follow-up to ensure effectiveness and adherence. |
