SAFETY OF PREOPERATIVE USE OF ANGIOTENSIN-CONVERTING ENZYME INHIBITORS AND ANGIOTENSIN RECEPTOR BLOCKERS IN PATIENTS UNDERGOING CARDIAC SURGERY: A SYSTEMATIC REVIEW AND META-ANALYSIS OF THE LITERATURE

Popis: differences in rates of in-hospital mortality (3.6% vs. 3.5%, p1⁄4 0.95) or morbidity (59.2% vs. 54.7%, p 1⁄4 0.36) were noted. After adjusting for baseline differences, ICS emerged as an independent predictor of reduced perioperative transfusion rates of pRBC (OR 0.56, 95% CI 0.32-0.96), coagulation products (OR 0.40, 95% CI 0.22-0.73) and any blood product (OR 0.50, 95% CI 0.29-0.84). CONCLUSION: The use of ICS in cardiac surgery is an effective way to reduce rates of perioperative blood product transfusion with no associated increase in postoperative bleeding or other complications. We recommend that ICS be implemented during all cardiac surgical procedures.
