A 60-year-old man suffering from angina had to undergo percutaneous transluminal coronary angioplasty (PTCA). In the pre-operative screening procedure anti-Vel antibodies were identified. The patient's blood group is O, rr. The prevalence of the blood group O, rr Vel(-) is 1:60,000. Compatible blood groups were not found in the patient's family. The only alternative method to give this patient the necessary units of blood was an auto-transfusion procedure. Because of the patient's clinical condition, however, it was necessary to use an erythrocytapheresis procedure, without changing his blood volume. We performed the erythrocytapheresis using the Fenwal CS-3000. Blood (920 mL) was collected, Ht 0.67 L L , this was equivalent to 3 units of packed cells compared with random donor blood. The erythrocytapheresis procedure was performed without any problem and the PTCA was done without complications or transfusion. The blood is stored in liquid nitrogen in the central laboratory of blood transfusion, Amsterdam. |
