Popis: | Background: ILDs are heterogeneous disorders with similar radiologic and clinical features but different pathogenesis, prognosis and therapeutic options. New markers are needed to optimize their treatment and management. Aim of the Study: To evaluate the role of different biomarkers: Myeloperoxidase (MPO), Human Neutrophil Peptides (HNP) and Eosinophil Cationic Protein (ECP) in ILD discrimination and course prediction. Methods: the prospective study enrolled 12 healthy individuals (HS), 55 patients with sarcoidosis, 22 with CTD-ILD and 24 IPF, measuring MPO, HNP and ECP in BAL. Patients were evaluated for 52 weeks to estimate pulmonary worsening and identify a biomarker predictive for disease behavior. Results: MPO was significantly higher in patients with sarcoidosis (29.2 pg/ml) and CTD-ILD (28.9 pg/ml) as compared to HS (17.3 pg/ml) and IPF (2.4 pg/ml), p Conclusions: MPO and HNP may have a role in discriminating ILD and in predicting IPF behavior, while ECP may be effective in predicting sarcoidosis pulmonary function worsening. |
