Breast preserving surgery for early stage breast cancers has been increasingly performed in Japan, but there are still many problems for indication. A 68-year-old female with papillotubular breast cancer was considered a candidate for breast preserving surgery from its tumor size and location which suited our standard indication. The patient had past history of bloody nipple discharge, however, frequent cytologies of bloody nipple discharge for three years revealed no malignant findings. The tumor, which appeared three years after nipple discharge ceased, was 2cm in diameter and located in lateral-upper quadrant having 4cm distance from lateral margin of the nipple. Quadrantectomy was performed and histological findings showed that cancer cells should be in the residual tissues. Thus Auchincloss' mastectomy was followed. For better decision-making of the indication for breast preserving surgery, further investigations of tumor size, histological type, or necessity of radiation therapy to the residual breast tissues are needed. |
