Popis: | Conventionally, congestive heart failure (CHF) has been treated with digitalis glycosides and diuretics to improve cardiac performance and reduce symptoms of congestion [1–3]. Recently, treatment of CHF has been advanced through the use of vasodilators and in particular angiotensin-converting enzyme inhibitors (ACEs) [4–6]. In addition to improving exercise tolerance and symptoms. a controlled clinical trial in severely disabled (NYHA class IV) CHF patients found a reduction in mortality when enalapril was added to various treatment regimens [7]. Mortality was also shown to be reduced in patients treated with the vasodilator hydralazine in combination with nitrates but not the alpha-blocker prazosin [4]. As the role of vasodilation in CHF has become more clear, the role of inotropy has become increasingly controversial. |
