Popis: Catheter-induced aortic dissection is a rare but potentially catastrophic complication of cardiac catheterization procedures. Injury to the aorta is often initiated in the ostium of a coronary artery and may extend variably in an antegrade or retrograde fashion. Risk factors for the development of catheter-induced aortic dissection are different from spontaneous aortic dissection. This point highlights the differences in the pathophysiology of the two conditions, which is also reflected in the contrasting management strategies. Catheter-induced aortic dissection is most often recognized on angiography during the index cardiac procedure and has been classified based on the extent of involvement of the aorta. Several factors affect the management strategy including the hemodynamic stability of the patient, propagation and extent of the aortic injury, presence of aortic valve incompetence, presence of pericardial effusion or cardiac tamponade, and the condition of the involved coronary artery. Current options for management include a conservative approach with careful surveillance, percutaneous stenting of the dissection entry point, and surgery. In many cases, catheter-induced aortic dissection can be effectively managed with stenting of the dissection entry point. Recent case series suggest an excellent short- and long-term prognosis when compared to spontaneous aortic dissection. In this book chapter, we aim to discuss the incidence, risk factors, clinical presentation, management of catheter-induced aortic dissection.
