The authors report their experience on GERD and hiatal hernia repair with a personal-shaped mesh. This mesh is composed by two layers, polypropylene and PTFE, joined together and tailored like a “C”. We decided to use this particular shaped mesh after having studied a mathematical and geometrical approach on the physiology of the crura. In fact, we realised that the strengths applied to the pillars generate two vectors directed inside. Otherwise, when you repair the hiatus in traditional manner, the vectors are directed outside and we have mediated the tension-free repair of inguinal hernia to perform a safe hiatoplasty. |
