REMEDIZER- An Innovative Program of Remote Home Care for Patients with Implanted Mechanical Heart Support. Single Centre Experience

Popis: Purpose To evaluate our single centre experience with REMEDIZER. This locally developed application is used to monitor patients with implanted heart assist pumps (LVAD, Left Ventricular Assist Device) after completed initial hospitalization. Methods REMEDIZER is an web and mobile based application. LVAD patients log there every day and store clinical data. Following data is saved: blood pressure, heart rate, body temperature, fluid intake and output, LVAD pump speed, flow, pulsatility index and power. Additionally, the patients store their clinical symptoms and introduce current drugs by scanning of the package. Encrypted data from the user's device is sent securely to the server via WiFi or GSM. The LVAD centre gets detailed information about the patient with history of results with the possibility of graphical analysis The treatment coordinator is automatically notified about new records with an notification and alarm system. The patient and the coordinator and other participating physicians can communicate using video chat. Results From December 2013 to October 2018 n=42 patients received an LVAD implantation at our institution (22 HeartWare, 5 HeartMate II and 15 HeartMate III systems). The system was used in n=37 patients (85,7%). During their LVAD therapy all patients used REMEDIZER regularly. In 100% of the cases the cause for use cessation was heart transplantation (n=8) or death (n= 5). Usually the patients log three times a day to the application. Actually n=24 (100% of patients at home) patients use the application. Conclusion We find REMEDIZER as a simple and attractive application for LVAD patients. It enables coordination of treatment by integrating data from various sources (patient, hospital) regarding all of the patient's ailments. It is just one application giving the LVAD centre detailed information with possibility of audio or video communication with alarm system. All of our LVAD patients use it at home regularly.
