A 3-year-old Caucasian twin boy (A) presented to the emergency department (ED) because of lethargy, nausea, and vomiting for 3 days. He had a 2-week history of polyuria, polydipsia, and 11 lb weight loss. His past medical history was significant for prematurity (27 weeks estimated gestational age), apnea of prematurity, gastro-esophageal reflux, constipation, and excessive salt craving. Family history was positive for type 1 diabetes (T1D) in a maternal cousin. On arrival to the ED, the patient was lethargic and moderately dehydrated; his pulse was 134, blood pressure was 80/48 mmHg, RR 30. The skin was slightly tan in color. Physical exam was normal. |
