SPECIAL FEATURES OF FLUID RESUSCITATION IN CASE OF A MULTIPLE ORGAN DAMAGE (LITERATURE REVIEW)

Popis: Review of the literature is devoted to multiple organ dysfunction syndrome and basic aspects of pathogenesis. Attention is concentrated on the special features of intenstive care. The paper presented the development of multiple organ dysfunction syndrome and place of sorbitol drugs at the stages of its development. Due to the colloid and crystalloid components, sorbitol agents (sorbilact and reosorbilact) promote detoxification, increasing blood volume, correction of water - electrolytic and acid-base balances, improve metabolism and blood rheology. When sorbitol is involved in the process of metabolism it results in fructose formation used for glycogenesis and glycolysis. Sorbitol possesses higher antiketogenic properties than fructose, it stimulates the oxidation of fatty acids by nonketogenic metabolism and promotes the use of ketone bodies in the Krebs cycle. It has a positive effect on the cardiac activity, regenerative and respiratory functions of the blood, stimulates mononucleolarghagocytic system, has detoxification effect, increases urine output, improves liver and kidney function. Due to the impact on the neuro-receptor apparatus and autonomic ganglia, it also intensifies synthesis of "hormonal secrets" that are formed in case of fructose decay, increases the secretion of villikinin, cholecystokinin, tensifies the synthesis of B vitamins, reduces the activity of translocation of the intestinal flora, optimizes the splanchnic blood flow, accelerates the repair of the intestinal wall cells, prevents the development of the lesions of the stomach and intestines.Conclusion: sorbitol drugs are expedient to be used in complex intensive care at all stages of multiple organ dysfunction syndrome.
