FOUR CASES OF TRAUMATIC CENTRAL RUPTURE OF THE LIVER

Popis: We have experienced 4 cases of central rupture of the liver attributed to traffic accidents. Three were males and one was female, and their ages ranged from 9 to 48 years. Diagnosis and post-traumatic observation were performed by CT and US. Three cases were treated conservatively since no signs of hemorrhagic shock were found. In one emergent lateral segmentectomy and mattress sutures of medial segment were performed to prevent bleeding caused by lacerations of the lateral and medial segments. But intrahepatic hematomas in the right lobe and the medial segment were not treated. However, immediately after the operation a hematoma in the right lobe close to the portal branch was ruptured to the abdominal cavity and partial liver resection was added. On the 11th postoperative day, a hematoma in the medial segtment was ruptured to the cut edge of the lateral segment and the similar procedure of the second operation was performed. Intrahepatic hematoma may be almost conservativelytreated but there remain possibilities to have rupture immediately or delayed as reported here. Careful observations of the vital signs and those with CT and US are necessary to decide the indication of operation.
