HLA IMMUNIZATION IN KIDNEY TRANSPLANTATION - COMPARISON OF RESULTS OBTAINED BY TWO METHODS

Popis: Determination of HLA antibodies presence and specificity is an immunological test used for patients awaiting organ transplantation. HLA immunization is detected by complement-mediated lymphocytotoxicity method (CDC) and solid phase method (Luminex). Aim of this study was to compare results obtained by both methods for 270 patients on the kidney transplantation waiting list. Negative result was shown by both methods for 162 (60%) patients. Positive result was obtained for 108 patients: both methods tested positive for 48 patients (18%), 57 patients (21%) were positive only with Luminex method and for 3 patients (1%) antibodies were detected only by CDC method. HLA immuization by CDC was demonstrated in 51 (19%) patients and by Luminex in 105 (39%) patients. High immunization was determined for 22 (8%) patients by CDC method, and for 78 (29%) patients by Luminex method. CDC method detected only HLA class I antibodies in 48 patients. Conversely, Luminex method detected only HLA class I antibodies in 25 patients, HLA class II antibodies in 13 patients, while 70 patients had both HLA class I and class II antibodies. IgM antibodies were detected by CDC method in a total of 10 patients out of whom 7 patients also demonstrated the presence of IgG antibodies.
