In this paper, we shall review 131 cases of carcinoma of the esophagus and cardia from three institutions covering a 10 year period from 1962 to 1972. Wide variations and approaches to the problems proved an opportunity to evaluate the role of various approaches to the quality of survival. We found that such procedures as feeding gastrostomy and cancer dilatation have little if any place for effective management. Direct, effective preparation, resection, and replacement provided the best quality of survival when appropriate. Procedures that did not relieve esophageal obstruction and prevent tracheoesophageal fistula were undesirable on the bases of both quality and longevity of survival. Management which delayed the removal of the obstruction allowed further deterioration of the patient's condition, in many instances placing the patient in the category of being preterminal. |
