Positive margins and residual disease following radical prostatectomy prediction and therapy

Abstrakt: Positive margin or residual disease after radical prostatectomy shows that complete eradication of tumor has not been achieved. This review of published data synthesizes the risk factors and prognosis of positive margins. Clinical stage, extracapsular invasion, and cancer volume are probably the major risk factors. Preoperative Gleason score and number of positive biopsy sites should also be considered in prediction of positive margin. Positive margin should be included in the prognostic evaluation, but its influence has to be moderated when other prognostic factors are associated, such as high Cleason score or extracapsular extension.
