Complex odontomas are benign lesions characterized by an amorphous mass of mineralized tissue, typically identified through routine imaging examinations. In this case, a 33-year-old female patient was referred to the oral and maxillofacial surgery service after a routine panoramic radiograph detected an asymptomatic lesion in her mandible. During the patient's medical history, she reported being a former smoker, and the clinical examination revealed an increase in bone volume in the right mandible, which was sensitive to palpation. The panoramic radiograph showed a radiopaque area measuring 2 cm in the right mandibular ramus. The preliminary diagnosis of a complex odontoma was subsequently confirmed through an excisional biopsy and histopathological analysis. The patient was lost to follow-up but returned to the service after one year, complaining of pain in the operated region. A follow-up panoramic radiograph revealed the presence of a residual lesion in the right mandibular ramus. Another surgical intervention was planned, and the patient continues to be monitored.
