The article discusses Baylor Health Care System's (BHCS) diabetes management program at their Diabetes Health and Wellness Institute. The author describes the importance of health education, the promotion of patient self-management and the need to involve family members in diabetes care. BHCS created both an inpatient and outpatient diabetes management program which used a holistic health equity model to identify patient health needs and promote positive treatment outcomes. |
