Off-label uses of the Amplatzer Piccolo Occluder in children with congenital and acquired heart diseases.

Abstrakt: Aim: The Amplatzer Piccolo Occluder (APO) is approved for patent ductus arteriosus (PDA) occlusion in infants weighing >700 g but could offer versatility to treat other lesions. Methods: Retrospective review of children in whom APO was utilized for defects other than PDA between January 2022 and June 2023. Results: The APO was used in nine patients; three for ventricular septal defects, four with coronary fistulas, one for a ventricular pseudoaneurysm and one where APO deployed within a fenestration of a previously placed Amplatzer Septal Occluder. All nine patients had successful occlusions without complications. Conclusion: The APO is a versatile device that can be used to treat various small diameter lesions in children besides the PDA for which it is currently approved for. Article highlights The Amplatzer Piccolo Occluder (APO) is currently used for closure of patent ductus arteriosus in infants and children. Its utility to treat other small lesions in children has not been previously described. In this series, the APO was used to treat small lesions such as coronary artery fistulas, residual ventricular septal defects, etc. All lesions were successfully treated using the APO, and there were no procedural complications. The APO can be used to successfully occlude small defects of many varieties in infants and children. The advantage of delivering a disc device through a relatively small catheter may prove valuable while treating certain vascular lesions in infants and small children. The 4-French delivery system allows this device to be used in small patients that cannot tolerate the large delivery systems of other devices. The symmetric design of the APO also allows for both antegrade and retrograde deployment. [ABSTRACT FROM AUTHOR]
