This article, published in the journal "Ultrasound in Obstetrics & Gynecology," examines the clinical significance of isolated lagging abdominal circumference (AC) in twin pregnancies. The study, conducted from 2010 to 2019, included 708 twin pregnancies. The primary outcome was small-for-gestational-age (SGA) infants at birth, and secondary outcomes included gestational age at birth and maternal characteristics. The study found that the rate of SGA was higher among pregnancies with isolated AC < 10th percentile compared to fetuses with overall estimated fetal weight (EFW) < 10th percentile. However, the study also found high rates of false-negative and false-positive diagnosis of SGA in twin pregnancies with an ultrasound diagnosis of fetal growth restriction (FGR). The authors suggest that future studies should consider incorporating other factors, such as umbilical artery Doppler assessment, to better assess the risk for SGA in twin pregnancies.
