Resolving the Dilemma on Continuous vs Intermittent β-Lactam Antibiotics in Sepsis.

Abstrakt: The article discusses the use of continuous versus intermittent infusion of β-lactam antibiotics in the treatment of sepsis. The Surviving Sepsis Campaign guidelines recommend prolonged infusion, but the evidence supporting this recommendation is moderate. The article presents the results of the Beta-Lactam Infusion Group (BLING) III trial, which found that continuous infusion of β-lactam antibiotics was associated with a 1.9% reduction in mortality compared to intermittent infusion, although this reduction was not statistically significant. A meta-analysis of 18 randomized controlled trials also supported the use of prolonged infusions. However, individual trials have not consistently shown improved clinical outcomes with continuous infusion.
